3. Patient Responsibilities

As a patient of Heal Health Hubs, you agree to:

• Inform us of any changes in your health condition, medications, or insurance coverage
• Arrive on time for scheduled appointments or notify us at least 24 hours in advance of cancellations
• Follow treatment plans and recommendations provided by our healthcare professionals
• Treat all staff members with respect and courtesy
• Pay for services rendered according to our billing policies

4. Appointment & Cancellation Policy

Appointments are reserved specifically for you. We understand that emergencies arise, but please provide at least 24 hours' notice if you need to cancel or reschedule. Failure to provide adequate notice may result in a cancellation fee of $50. Repeated no-shows may result in dismissal from our practice.

5. Payment & Billing

Payment is due at the time of service unless other arrangements have been made. We accept:

• All major credit and debit cards
• Cash and checks
• Health Savings Accounts (HSA) and Flexible Spending Accounts (FSA)
• Most major insurance plans (subject to verification)

Patients are responsible for any co-pays, deductibles, and non-covered services. Outstanding balances may be subject to collection procedures.

6. Insurance

We accept most major insurance plans. It is the patient's responsibility to verify coverage before receiving services. We will submit claims on your behalf, but ultimate responsibility for payment rests with the patient if insurance denies coverage.
